Procedure
Part A: Performance Objective Plan (POP) & Assessment
Employee
- Prepare status updates on your performance objectives
- Compile any supportive information you may have
Supervisor
- Schedule meeting with employee and request employee submission of status updates, etc..
- Compile any relvevant information you may have on employee objectives
- Review what has been accomplished against agreed upon objectives and assign a rating per objective
Part B: Competency Assessment
Employee
- Complete the Competency Assessment on yourself
- Compile any supportive information you may have to support your assessment for each competency
- Plan to discuss with your supervisor
- Think about areas to develop that you might want to discuss for the next Performance Planning meeting
Supervisor
- Complete the Competency Assessment on your employee
- Compile any supportive information you may have to support your assessment for each competency
- Plan to discuss with your employee
- Think about any areas of development you might want to discuss with your employee that you might want to include for the next Performance Planning meeting
Part C: Summary
Employee
- Complete Other Contributions you may have made
- Meet with supervisor on final assessment
- After Competency Assessment is discussed, check acknowledgement box
- Complete employee comments
- Sign the form as validation that the final assessment has been discussed and seen by you (this does not mean agreement)
- Make copies of signed original for both your supervisor and yourself
Supervisor
- Assign an overall rating based on performance objectives and competency assessment.
- Add any Other Contributions of your employee that you may have noted in the past year
- Complete supervisor comments
- Sign the form and meet with second level reviewer for approval and signature
- Meet with employee for Final Assessment
- After Competency Assessment is discussed, check acknowledgement box
- Obtain employee signature and comments
- After copies are made, turn completed form into your Business Affairs Officer or Human Resources/Employment Services Center (find out what is appropriate for your area)
