Phase Three - Final Assessment
Part A: Performance Objective Plan (POP) & Assessment
Employee starts the process
- Prepare a list of achievements regarding your performance objectives and submit to your supervisor
- Complete other contributions section
- Provide employee comments
Supervisor
- Review what has been accomplished against agreed upon objectives and assign a rating per objective
- Review other contributions section
- Provide supervisor comments
Part C: Employee Development Plan (EDP) & Assessment
Employee
- Prepare a list of achievements regarding your development plan objectives and submit to your supervisor
- Complete employee comments
Supervisor
- Review what has been accomplished against agreed upon development plan and assign a rating per objective
- Complete supervisor comments
Part D: Summary
Employee
- Meet with supervisor on final assessment
- Complete employee comments
- Sign the form as validation that the final assessment has been discussed and seen by you
- Make copies for both your supervisor and yourself
Supervisor
- The overall rating will be calculated on the form. You may override the calculated rating by typing over the letters, but you must include an explanation in the Supervisor' Summary of Performance Comments of why you overrode the rating. If you overrode the rating, you will not see the calculated rating again so you need to ensure that you write down the calculated rating before you change it, in case you want to use the calculated rating instead. If you don't override the rating, still provide a summary comment
- Sign the form and meet with second level reviewer for approval
- Meet with employee for Final Assessment
- After copies are made, turn completed form into Human Resources/Employment Services Center
