LEADS Form Guide

For Non-Represented Academic Administrators:

Look for the Academic Administrator Form and Academic Administrator Guidelines here.

For All Other Non-Represented Employees:

We have noticed that on the last page of the old Wayne LEADS form, when the supervisor or employee comments expands, it has been overlapping information below the box. As a temporary fix, we have limited expansion of the supervisor and employee comments on the last page used for the final assessment. We have fixed this issue on the new Wayne LEADS Form. For Performance Year 2012 we have updated the form to reflect feedback recieved by users of the process:

  • Added Job/Functional Skills as a Core Competency
  • Moved Business Acumen from Core Competencies to Supervisor/Manager Competencies
  • Removed Leadership from Supervisor/Manager Competencies
  • Fixed the expansion of the Supervisor and Employee Comments on the last page

Once the form is opened...

  1. Save it to your own local drive.
  2. Call it what you want but we recommend using your first name and first initial of your last name with LEADS for easy identification, (example: FirstNameLastInitialLEADS.pdf).
  3. Now instead of using the form from the this website, open your saved form from your computer.
  4. Once the form is open from your computer, you can start entering information into the form. You can save it and reopen later or save it and E-mail as an attachment.

The new form will work exactly the same way).
